Ohio Living is seeking an RN Case Manager for Home Health to plan, coordinate and deliver high-quality home health services. You will ensure compliance with company standards and regulatory guidelines while developing comprehensive care plans for patients in homes and facilities.

The role requires one year of acute care or medical-surgical nursing experience, Ohio RN license, and proficiency with Windows/Microsoft Office; EMR experience is a plus.

Formación

• Current unencumbered Ohio RN license to practice.
• One year of acute care or medical-surgical nursing experience required.
• Medicare-certified home health agency experience preferred.
• Proficiency with Windows and Microsoft Office; EMR experience preferred.

Responsabilidades

• Care management and service delivery in patients' homes or facilities; develop care plans.
• Monitor patient needs and adjust care plans; communicate with team leaders.
• Quality and compliance activities; maintain accurate patient records.
• Education for patients and caregivers; ensure professional standards.
• Coordinate with other providers to ensure continuity of care.
