Job summary

Qualifications

• Current unencumbered Ohio RN license required.
• OASIS experience is required.
• Proficiency with Windows, MS Word/Excel/PowerPoint and Internet access essential.

Responsibilities

• Care management and service delivery for patients at home or in facilities (70%).
• Develops and implements comprehensive care plans per patient needs.
• Audits patient records and ensures regulatory compliance and quality.
• Accurate documentation and coordination with team members.
