Job summary

Visiting Nurse Health System is seeking a Registered Nurse Case Manager for the Conyers region to coordinate comprehensive home health care. You will assess, plan, implement, monitor, and evaluate patient care while engaging families in education and discharge planning.

Ideal candidates hold a GA RN license with CPR, valid driver’s license, and home health experience. You will work with an interdisciplinary team, supervise LPNs/HHAs, and participate in weekly team conferences.

Qualifications

• RN licensed in Georgia with current CPR.
• Must possess valid driver's license and reliable transportation.
• Knowledge of Medicare/Medicaid rules as applied to home health care.
• Strong clinical nursing skills and ability to work with interdisciplinary teams.
• Ability to work independently in a home-based environment and educate patients and families.

Responsibilities

• Develop and implement individualized plans of care based on physician orders and patient needs.
• Modify care plans as conditions change and reassess effectiveness.
• Educate patients and families on diseases, self-management, and hospitalization prevention.
• Complete OASIS documents and visit notes within VNHS guidelines.
• Communicate patient status changes clearly to patients, physicians, and team members.
• Provide emotional support and connect patients with MSW and community resources as needed.
• Supervise LPNs and HHAs per VNHS policy and ensure care plans meet patient needs.
• Participate in SOC/ROC calls and weekly team conferences.
• Deliver high-quality home-based care in line with VNHS standards.
