Job summary

Visiting Nurse Health System is seeking an experienced RN Case Manager for Hospice in Buckhead, GA. This full-time role coordinates care for a caseload of home health patients, ensuring quality and adherence to utilization guidelines.

You will assess, plan, implement, monitor, and evaluate comprehensive care plans, while educating patients and families and collaborating with the interdisciplinary team in a home-based environment.

Qualifications

• Graduate from an accredited nursing program (Diploma, ASN, or BSN preferred).
• Current Georgia RN license and CPR certification required.
• Experience in home health and hospice care strongly preferred.
• Knowledge of Medicare/Medicaid regulations for home health care.

Responsibilities

• Coordinate care for a caseload of home health patients.
• Assess, plan, implement, monitor, and evaluate comprehensive care plans.
• Educate patients and families on disease processes and self-management.
• Complete OASIS documents, visit notes, and required records timely.
• Communicate patient status changes to physicians and team members.
• Supervise LPNs and HHAs and participate in SOC/ROC calls.
• Deliver high-quality care in the home and meet nursing standards.
