Job summary

Visiting Nurse Health System is seeking a Registered Nurse Case Manager for Home Health in Georgia to coordinate care for a caseload, ensuring care meets quality standards and utilization guidelines. The RN Case Manager assesses, plans, implements, monitors, and evaluates plans of care to ensure optimal patient outcomes in the home setting.

This role requires a compassionate, skilled nurse who thrives in a home-based environment and values patient education and family engagement.

Qualifications

• Graduated from an accredited nursing program and hold current Georgia nursing licensure.
• CPR certification is required and essential for patient safety.
• Strong knowledge of Medicare/Medicaid regulations as applied to home health care.
• Ability to manage time effectively and assess patients independently.

Responsibilities

• Develop and implement individualized plans of care by assessing patient needs with the interdisciplinary team.
• Modify and update care plans as patient conditions change and evaluate effectiveness.
• Educate patients and families on disease processes and self-management.
• Complete and submit OASIS documents and visit notes within VNHS timeliness guidelines.
• Communicate changes in patient status to physicians and team members, and document appropriately.
• Offer emotional support and connect patients/families with MSW and community resources.
• Supervise LPNs and HHAs per VNHS policy.
• Participate in SOC/ROC conference calls and weekly team conferences.
• Deliver high-quality care in the home, adhering to VNHS policies and nursing standards.
• Maintain required competencies through in-services and continuing education.
• Schedule patient visits efficiently and manage caseload to optimize productivity.
• Precept newly hired nursing staff and communicate with office staff regarding patient status.
