Job summary

Visiting Nurse Health System is seeking an RN Case Manager for Home Health and Hospice in Marietta, GA. You will coordinate care for a patient caseload, ensure quality and utilization standards, and educate families in a home-based setting.

The role requires GA RN licensure, CPR certification, and strong collaboration with an interdisciplinary team to optimize patient outcomes. This is a full-time position with weekend on-call rotation.

Qualifications

• Graduated from an accredited nursing program.
• Hold current licensure by the Georgia State Board of Nursing and CPR certification.
• Have knowledge of Medicare/Medicaid and home health funding rules.

Responsibilities

• Develop and implement individualized care plans with physicians and the interdisciplinary team.
• Provide patient and family education on disease processes and self-management.
• Complete and submit OASIS and visit documentation within VNHS timelines.
• Supervise LPNs and HHAs per VNHS policy and participate in SOC/ROC calls.
• Deliver high-quality care in the home and maintain nursing competencies.
