The position requires an active Georgia RN license and CPR, with prior home health experience preferred. You will work in Decatur, GA, with a flexible full‑time schedule including weekend/holiday on‑call rotation.

Qualifications

• Graduate from an accredited nursing program (Diploma/ADN/BSN preferred).
• Hold current Georgia RN licensure and CPR certification.
• Proven ability to plan, coordinate, and educate patients and families.

Responsibilities

• Coordinate care for a home‑health caseload and ensure quality care.
• Assess, plan, implement, monitor, and evaluate plans of care.
