Job summary

Qualifications
- Graduate from an accredited nursing program (Diploma, Associate, or BSN).
- Hold current Georgia RN license and CPR certification.
- Valid driver's license with reliable transportation.
- Home health experience preferred for PRN positions.
- Knowledge of Medicare/Medicaid home health regulations.
Responsibilities
- Coordinate care for a patient caseload in home health.
- Assess, plan, implement, monitor and evaluate care plans.
- Educate patients and families on disease management and self-care.
- Complete OASIS and required visit documentation timely.
- Communicate status changes with care team and physicians.
- Supervise LPNs and HHAs per VNHS policies.
- Participate in SOC/ROC calls and weekly team meetings.
- Deliver high-quality care in patients' homes and maintain competencies.
