Job summary

Visiting Nurse Health System (VNHS) is seeking an experienced RN Case Manager for Home Health in Conyers, GA. You will coordinate care for a caseload, ensuring quality standards and regulatory compliance, and work to achieve optimal patient outcomes.

The role requires leadership in developing care plans, educating patients and families, and collaborating with interdisciplinary teams. A GA nursing license and CPR certification are required, along with strong clinical and documentation skills.

Qualifications

• Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).
• Hold current licensure by the Georgia State Board of Nursing and valid CPR certification.
• Possess a valid driver’s license, reliable transportation, and current auto insurance.
• Have prior home health experience if applying as PRN staff.
• Demonstrate knowledge of Medicare, Medicaid, and funding source regulations as applied to home health care.
• Exhibit advanced clinical nursing skills and comfort collaborating with interdisciplinary teams.
• Have proven decision-making, time management, and interpersonal skills, functioning independently in a home environment.
• Are proficient with computers and basic technology for documentation, email, HR systems, and online training.

Responsibilities

• Develop and implement plans of care with physician orders after patient assessment.
• Modify and update care plans as conditions change, evaluating effectiveness.
• Educate patients and families on disease processes and self-management.
• Complete and submit OASIS documents, visit notes, and records on time.
• Communicate patient status changes to physicians and team; document accordingly.
• Provide emotional support and connect to MSW and community resources.
