Visiting Nurse Health System is seeking an RN Case Manager for Home Health serving Lithonia and Ellenwood, GA. Qualifications

• Graduate from an accredited nursing program; GA RN license and CPR required.
• Strong clinical skills with ability to manage client relationships in home health.
• Senior-level RN with patient transition experience preferred.

Responsibilities

• Develop and implement individualized care plans with physician orders.
• Modify plans as patient conditions change and reassess outcomes.
• Educate patients and families on disease management and prevention.
• Complete OASIS documents and visit notes per guidelines.
• Communicate changes to patients, physicians, and team; document appropriately.
• Provide emotional support and connect to MSW and community resources.
• Supervise LPNs and HHAs per VNHS policy.
• Participate in SOC/ROC calls and weekly team conferences.
• Deliver high-quality care in home settings; adhere to policies.
• Maintain competencies via in-services and continuing education.
• Schedule visits efficiently and manage caseload.
• Precept new nursing staff and coordinate with office staff.
