Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate all aspects of patient home care. You will conduct admission visits, assess eligibility for home health services, and develop the plan of care in collaboration with the patient and medical team.

The ideal candidate is a graduate of an accredited nursing program, holds an active RN license, has a valid driver's license, and is ready to provide compassionate, patient-centered care in the community.

Qualifikationen

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• One year’s experience as a Registered Nurse, two years preferred.

Aufgaben

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visits and determine eligibility for home care services and develop the plan of care.
