Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate all aspects of the patient’s home care. You will interact with other medical disciplines as needed and provide professional care to the patient.

You will also perform the admission visit to the patient in the home and determine the patient’s eligibility for home care services and develop the plan of care to be followed. The RN must be a graduate of an accredited nursing program, hold an active RN license in the

Competenze

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current RN license in the state.
• One year of RN experience; two preferred.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Mansioni

• Coordinate all aspects of the patient’s home care as RN Case Manager.
• Interact with other medical disciplines as needed to provide care.
