Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with medical disciplines as needed and provide professional care to the patient in the home.

The role includes admission visits to determine eligibility for home care services and to develop the plan of care to be followed. Requires RN licensure, a valid driver’s license, and CPR/Hepatitis clearance.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
