Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate all aspects of home-based patient care. You will perform admission visits, assess eligibility for home care services, and develop individualized care plans in collaboration with the care team.

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Active RN license in the state of practice.
• Valid driver's license and CPR certification or proof of CPR.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform the admission visit to the patient’s home.
• Determine eligibility for home care services and develop the plan of care.
• Collaborate with other medical disciplines to ensure quality patient care.
