Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with other medical disciplines, perform admission visits, assess eligibility for home care, and develop the plan of care to be followed in the home.

Qualifications include RN licensure, a current driver’s license, CPR, and reliable transportation. Prior home health experience is valued, with opportunities for career advancement and comprehensive benefits.

Qualifications

• Graduate of an accredited nursing program (diploma, ADN or BSN).
• Current state license as a Registered Nurse.
• Current state driver’s license.
• One year’s RN experience (two preferred).
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as an RN Case Manager.
• Perform admission visits and determine eligibility for home care services.
• Develop and follow the plan of care with other medical disciplines.
