Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home care. You will conduct admission visits, assess eligibility, and develop a comprehensive plan of care for patients in their homes.

The role involves collaborating with medical disciplines, ensuring high-quality nursing care, and maintaining compliance with regulations. A valid RN license, driver’s license, reliable transportation, and CPR are required.

Qualifications

• RN case management and direct patient care in home health.
• CPR certification and Hepatitis consent/declination on file.
• Valid driver license and reliable transportation required.
• One year of RN experience preferred, two years preferred in some cases.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform admission visits to determine eligibility and plan of care.
• Collaborate with other medical disciplines as needed to provide patient care.
• Ensure compliance with regulatory requirements and documentation.
