Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate all aspects of a patient’s home care and provide professional nursing care in the home setting. You will conduct admission visits, determine eligibility for home health services, and develop the plan of care to be followed.

The role requires an active RN license in the state, a valid driver’s license, CPR, and reliable transportation.

Qualifications

• Graduate of an accredited nursing program (diploma/ADN/BSN).
• Current state RN license.
• Valid driver’s license.
• One year RN experience; two preferred.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsabilités

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform home admission visits and determine eligibility for home care; develop the plan of care.
