Job summary

UnitedHealth Group is seeking a Nurse Case Manager in Bangor, ME to guide patients and families through the care continuum, coordinate care, support discharge and transition planning, and manage ongoing outpatient care under supervision. You will educate, coach, and assist with treatment decisions while collaborating with a Care Advocate and Social Worker.

Responsibilities include patient assessments, care planning, referrals, staff education, and timely documentation.

Qualifications

• Current RN license, state(s) you are assigned to work.
• Valid driver's license, reliable transportation and liability insurance.
• Current CPR certification.
• 1+ years of clinical experience as an RN.

Responsibilities

• Assess patients and collaborate with care team to develop and implement a plan of care.
• Make referrals to other disciplines as indicated by patient needs.
• Promote patient health and independence through teaching and rehab measures.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Regularly communicate patient progress to clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and services as assigned.
• Participate in clinical development and continuing education.
