Job summary

UnitedHealth Group is seeking an RN Case Manager to guide patients and families through the care continuum, coordinating care, supporting discharge planning, and managing ongoing outpatient care. You will work under supervision of an RN or MD, educating and supporting care decisions while collaborating with a Care Advocate and Social Worker.

Primary responsibilities include assessments, referrals, student education, and on-call coverage, with opportunities to grow through clinical development

Qualifications

• Current RN license for the assigned state(s).
• 1+ year of clinical experience as an RN.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
