Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage outpatient care under supervision. You will educate, coach, and support decision-making while collaborating with Care Advocates and Social Workers.

You will perform assessments, coordinate referrals, educate patients, supervise nursing staff, document thoroughly, and participate in ongoing clinical development.

Qualifications

• Current RN license, state(s) you are assigned to work
• Valid CPR certification
• Valid driver's license, reliable transportation and liability insurance
• 1+ years of clinical experience as an RN

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by patient needs
• Promotes patient health and independence through teaching and rehabilitative care
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities as assigned
• Participates in clinical development and continuing education programs
