Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, and support discharge planning. Under the supervision of an RN or MD, you will help ensure patients receive appropriate care and provide education and decision-support.

Qualifications

• Current RN license valid for assigned state(s)
• 1+ year of clinical RN experience
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

Responsibilities

• Performs patient assessments and collaborates with the care team to develop a plan of care
• Makes referrals to other disciplines as indicated by patient needs
• Promotes patient health and independence through teaching and rehabilitation
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• On-call responsibilities and services to patients/families
• Participates in clinical development and continuing education
