Job summary

UnitedHealth Group is hiring a Nurse Case Manager in Portland, ME, to guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage outpatient care under supervision of an RN or MD.

You will help ensure appropriate level of care, provide education and coaching, and collaborate with a Care Advocate and Social Worker. Join a mission-driven team focused on delivering quality home-based care.

Qualifications

• Current RN license for the state(s) you are assigned to work.
• Current CPR certification.
• 1+ year(s) of clinical RN experience.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
