Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinate care, and support discharge planning. You will work under supervision to ensure appropriate care levels, provide education, and collaborate with care advocates and social workers.

Main responsibilities include patient assessments, referrals, education, staff supervision, documentation, and on-call duties.

Qualifications

• Current RN license for the assigned state.
• CPR certification is required.
• At least 1 year of clinical RN experience.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Assess patients and collaborate with care team to plan care.
• Refer to other disciplines as indicated by patient needs.
• Educate patients to promote self-care and independence.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Communicate patient progress to clinical manager and team.
• Plan and provide staff education.
• Participate in on-call duties and services.
• Engage in ongoing clinical development and training.
• Perform other duties as assigned.
