Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating care, supporting discharge planning, and managing ongoing outpatient care under supervision of an RN or MD.

Responsibilities include assessments, referrals, patient education, and collaboration with Care Advocates and Social Workers, with on-call duties and ongoing clinical development.

Qualifications

• Current RN license in the state(s) you are assigned to work.
• 1+ year of clinical experience as an RN.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Participates in on-call responsibilities and clinical development.
