Job summary

UnitedHealth Group in Virginia is seeking a Nurse Case Manager to guide patients and families through the care continuum under the supervision of an RN or MD. You will assess needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care.

You will collaborate with a Care Advocate and Social Worker, educate and coach patients, document promptly, and participate in ongoing professional development.

Qualifications

• Current RN license corresponding to assigned state(s).
• 1+ year of clinical RN experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Makes referrals as indicated by patient needs and documents rationale for non-referral.
• Educates patients and families, promoting independence and self-care.
• Supervises LPNs and HHAs.
• Documents care timely and accurately.
• Communicates patient progress to clinical manager and care team.
• Plans and provides staff education.
• Participates in on-call responsibilities.
