Qualifications

• Current RN license valid in the state(s) you work.
• 1+ year clinical RN experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health through teaching and rehabilitative measures; supports self-care learning.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Participates in on-call responsibilities.
• Pursues clinical development and continuing education.
