Qualifications

• Active RN license in the state you work.
• One year RN experience in home health or similar setting.
• Valid driver’s license and vehicle for travel to patient homes.

Responsibilities

• Deliver skilled, patient-centered care in home health in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Ensure accurate and compliant EMR documentation.
