Job summary

Graham Healthcare Group in Jermyn, PA is seeking a Registered Nurse (RN) for Home Health to provide skilled nursing care to patients in their homes.

This ongoing staff role offers a 40-hour work week, 8-hour shifts, and a first-year $10,000 success bonus, with opportunities for growth across our Home Health and Hospice teams.

Requirements include an active RN license in Pennsylvania, at least one year of RN experience, and a valid driver’s license with up to 20% travel to patient locations.

Qualifications

• Active RN license in the state you work
• Minimum of 1 year RN experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
