Qualifications

• Active RN license in the state you work.
• Minimum of 1 year RN experience delivering home health care.
• Valid driver’s license and the ability to drive ~20% of your workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, ensure thorough EMR documentation.
• Travel to patient homes within assigned territory.
