Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to provide skilled nursing care to patients in their homes in Monongahela, Pennsylvania. This full-time role offers 40 hours per week and an 8-hour shift, with bonuses potential as described.

You will collaborate with an interdisciplinary team, perform assessments, manage medications, educate families, and ensure compliant EMR documentation while traveling within the assigned territory.

Qualifications

• Active RN license in the state you work
• One year of RN experience in home health preferred
• Valid driver’s license and ability to drive ~20% of the time

Responsibilities

• Deliver skilled, patient-centered care in a home health setting
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically, solve problems in real time, and ensure thorough EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
