Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) for Home Health in Erie, PA. This ongoing staff position requires 40 hours/week, 8-hour shifts, and travel to patient homes within the territory. Weekend admissions are included with substantial bonus potential and comprehensive benefits.

Qualifications

• Active RN license in the state you work (PA).
• One year of RN experience.
• Valid driver’s license required.
• Must be able to travel to patient homes (approximately 20% of workday).

Responsibilities

• Deliver skilled, patient-centered care in home health settings with an interdisciplinary team.
• Conduct start-of-care assessments incl. clinical evals and medication reconciliation.
• Educate patients and families about admission processes and care plans.
• Coordinate admissions with accurate EMR documentation.
• Initiate evaluations to other disciplines when indicated.
• Travel to patient homes within assigned territory.
