Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) in Home Health for patients in the Washington, PA area. This ongoing role offers 40 hours per week, 8-hour shifts, and a first-year $10,000 success bonus with potential for growth across the organization.

You will deliver skilled, patient-centered care to homebound patients, conduct assessments, manage medications, educate families, and maintain compliant EMR documentation while traveling within your territory.

Qualifications

• Active RN license in the state you work
• Minimum one year RN experience in home health
• A valid driver’s license and ability to travel ~20% of the workday

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate families on disease management and treatment plans
• Ensure accurate EMR documentation and compliance
• Work independently, but collaborate with team and travel to patient homes
