Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health in the Dunbar, PA area to provide skilled nursing care in patients’ homes. This ongoing Staff position involves 40 hours per week, 8-hour shifts, and travel within the territory.

Eligible for a substantial bonus program totaling up to $25,000 over the first three years, plus comprehensive benefits and a strong field support structure for growth and continued professional development.

Qualifications

Active RN license in the state you work.
One year of RN experience in home health or equivalent.
Valid driver's license and ability to drive ~20% of workday.

Responsibilities

Deliver skilled, patient-centered home health care with an interdisciplinary team.
Perform patient assessments, medication management, and care planning.
Educate patients and families on disease management and treatment plans.
Document thoroughly in EMR and maintain compliance.
Travel to patient homes within assigned territory.
