Graham Healthcare Group is seeking a Registered Nurse (RN) for Home Health in Van Voorhis, PA. This staff position covers a 40-hour week with an 8-hour shift, delivering skilled, patient-centered care in patients' homes within your assigned territory.

You will perform assessments, care planning, medication management, and education, while coordinating with an interdisciplinary team. A valid RN license in Pennsylvania, at least one year of RN experience, and a valid driver’s license are required.

Qualifications

• Active Registered Nurse license in PA
• One year RN experience preferred
• Valid driver’s license required for home visits

Responsibilities
