Job summary
Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care and interact with other medical disciplines to provide professional care in the patient’s home. The nurse will perform admission visits, assess eligibility for home care services, and develop an individualized plan of care.
Qualified candidates have an accredited nursing diploma/degree, current RN license and driver's license, CPR, Hepatitis consent/declination, and
Qualifications
- Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
- Current state license as a Registered Nurse; valid Driver's License.
- One-year RN experience; two years preferred.
- CPR certification and Hepatitis consent/declination.
- Reliable transportation with auto liability insurance.
Responsibilities
- Coordinate all aspects of the patient’s home care.
- Interact with other medical disciplines as needed and provide professional patient care.
- Perform admission visits at the patient’s home and determine eligibility for home care services.
- Develop and follow the plan of care.
