Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate home-based patient care. You will perform admission visits, determine eligibility for home care services, and develop individualized plans of care with the interdisciplinary team.

Required RN license, CPR, and a valid driver’s license with reliable transportation. Prior RN experience is preferred; vehicle allowance is offered as part of the package.

Qualifications

• Current state RN license in good standing.
• One year RN experience preferred (two years preferred).
• CPR certification and Hepatitis consent/declination on file.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of home care for patients.
• Perform admission visit to assess eligibility for home care services.
• Develop and implement the plan of care with multidisciplinary team.
