Angels Care Home Health is seeking an RN Case Manager to coordinate home-based patient care in Odessa, TX. You will perform admission visits, assess eligibility for home care, and create a personalized care plan for each patient.

Qualifications

• Graduate of an accredited nursing program.
• Current state RN license.
• Valid driver's license.
• One year of RN experience; two years preferred.
• CPR certification.
• Reliable transportation.

Responsibilities

• Coordinate all aspects of the patient’s home care as the RN Case Manager.
• Collaborate with other medical disciplines as needed to provide professional care.
• Perform admission visits to patients in their home and determine eligibility for home care services.
• Develop and follow the plan of care tailored to each patient.
