Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care and collaborate with other medical professionals. The role includes performing admission visits, assessing eligibility for home care services, and developing individualized care plans in the patient’s home.

The ideal candidate holds an RN license in the state, possesses a valid driver’s license, and has at least one year of RN experience (two preferred).

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed to provide professional care.
• Perform admission visits to patients in the home and determine eligibility for home care services.
• Develop the plan of care to be followed.
