Job summary

Angels Care Home Health is seeking an Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will perform admission visits in patients’ homes, determine eligibility for home health services, and develop individualized care plans.

The role requires an active RN license, a valid driver’s license, reliable transportation, and CPR certification. Prior RN experience is preferred. We offer competitive benefits and growth opportunities within a national network.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed & provide professional care to the patient.
• Perform the admission visit to the patient in the home & determine eligibility for home care services & develop the plan of care.
