Job summary

Qualifications

• Graduate from an accredited School of Nursing.
• BLS required per policy guidelines.
• Current RN license in the state of work/residency or NP/temporary RN license as applicable.

Responsibilities

• Manage an assigned patient caseload with timely care and documentation.
• Collect data from home assessments, telephonic contacts, and EMR.
• Analyze data to identify issues and risks for high-quality outcomes.
• Prioritize critical situations to ensure patient safety and timely services.
• Identify barriers (language, literacy, etc.) and coordinate with leadership.
• Develop and communicate a plan of care to involved parties.
• Coordinate care with a multidisciplinary team; supervise LPNs and HHAs.
• Perform ordered treatments per state regulations and agency policy.
• Evaluate progress toward patient-centered goals.
