Job summary

Advocate Health is seeking an RN for case management in the Cabarrus region home health program. You will oversee a patient caseload, coordinate a multidisciplinary team, supervise LPNs and HHAs, and ensure timely documentation and adherence to the plan of care.

This full-time role requires BSN preferred, current RN license in North Carolina, BLS, and strong assessment, communication and critical thinking skills. Schedule is M–F 8:30am–5:00pm with occasional on-call.

Qualifications

• RN license in NC (or eligible) with current BLS.
• BSN preferred.
• Ability to perform home health patient assessments and documentation.

Responsibilities

• Manage daily patient caseload in home health with timely care.
• Coordinate data from assessments and EMR to identify risks and plan interventions.
• Supervise LPNs and HHAs and oversee care coordination with multidisciplinary team.
• Ensure plan of care is communicated and implemented effectively.
• Provide ordered treatments in accordance with policy and regulations.
