Job summary

Qualifications

Responsibilities

• Performs all scheduled visits and collaborates with the interdisciplinary team as necessary.
• Conducts comprehensive nursing assessments and updates the care plan accordingly.
• Communicates with physicians to confirm and update the treatment plan as needed.
• Monitors patient responses and revises care plans based on findings.
• Participates in discharge planning and utilizes community resources to ensure continuity of care.
• Documents patient records accurately in accordance with guidelines.
• Demonstrates problem-solving and clear communication with physicians and case managers.
• Adheres to policies, procedures, and safety guidelines.
• Performs patient positioning and lifting with proper technique as required.
• Provides age-appropriate care, understanding developmental needs across the lifespan.
