Qualifications

Responsibilities

• Manage an assigned patient caseload daily with timely care and documentation.
• Collects data from home assessments, telephonic contacts, and EMR.
• Analyze data to identify issues and resources, high-risk patients, and plan interventions.
• Prioritize issues based on patient health and safety.
• Develop and oversee an achievable plan of care communicated to involved parties.
• Coordinate care with multidisciplinary team; supervise LPNs and HHAs.
• Perform treatments, therapies, and procedures per regulations and policies.
• Evaluate progress toward patient-centered goals.
