Atrium Health is seeking a compassionate RN Case Manager to oversee a patient caseload in the home health setting, ensuring timely care and documentation per the plan of care and physician orders.

You will collect health data from home visits and EMR, analyze risks, coordinate a multidisciplinary team, supervise LPNs and HHAs, and ensure high-quality outcomes for patients with complex needs.

Formación

• Graduate from an accredited School of Nursing; BSN preferred.
• Current RN license in the state of work; MLS or temporary license acceptable per policy.
• BLS certification required.

Responsabilidades

• Manage daily patient caseload in home health per plan of care and MD orders.
• Collect health data from home visits, telephonic contacts and EMR.
• Analyze data to identify risks and coordinate case conferences for high-risk patients.
• Prioritize issues based on patient health, safety, and service impact.
• Identify language, literacy or other barriers and engage appropriate resources.
• Develop and communicate an actionable plan of care with the care team.
• Direct care coordination and supervision of licensed practical nurses and home health aides.
• Perform required treatments and therapies per policy and regulations.
• Evaluate progress toward patient-centered goals.
