UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge planning, and manage outpatient care under supervision, ensuring appropriate levels of care and providing education and decision support.

This role involves collaboration with a Care Advocate and Social Worker, supervision of LPNs and HHAs, and active participation in continuing education and program development.

Competenze

• Current RN license in the state(s) you are assigned to work.
• Valid CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Minimum 1 year of clinical experience as an RN.

Mansioni

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitation measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on‑call responsibilities and on‑call services.
• Participates in clinical development and continuing education programs.
