Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients through the care continuum, coordinate services, support discharge planning, and manage outpatient care under RN/MD supervision. You will help ensure appropriate level of care, provide education and decision support, and collaborate with a Care Advocate and Social Worker.

Primary responsibilities include patient assessments, referrals, education, supervision of LPNs and HHAs, timely documentation, and on-call services.

Qualifications

• Current RN license in applicable state(s)
• Current CPR certification
• Valid driver's license and reliable transportation
• 1+ year of clinical RN experience

Responsibilities

• Assess patients and collaborate with care team to plan care
• Refer to other disciplines as needed and document decisions
• Educate patients and families, promote self-care
• Supervise LPNs and HHAs
• Complete timely and accurate documentation
• Communicate patient progress to clinical manager and team
• Plan staff education
• Participate in on-call responsibilities
• Engage in clinical development and continuing education
