Job summary

Atrium Health Cabarrus - Case Management is seeking a registered nurse for full-time case management and care coordination. You will work with patients, families, and a multidisciplinary team to overcome barriers, ensure timely care, and maintain compliant documentation.

Requirements include a BSN, RN license, and 5 years of clinical nursing or 1–2 years in care management, plus Epic EMR experience and strong communication skills. Relocation not indicated; benefits provided.

Qualifications

• Bachelor's degree in nursing (BSN) required.
• RN license to practice in the state.
• Typically 5 years clinical nursing or 1–2 years in care management.
• Experience with Epic EMR strongly preferred.
• Excellent communication, organization, and problem-solving skills.
• Ability to travel between clinic sites as needed.

Responsibilities

• Facilitate communications among patient, family, multidisciplinary team, and care management to resolve barriers to timely care delivery.
• Collaborate with patient/family and care teams to provide high-quality, cost-effective healthcare services.
• Use evidence-based approaches to engage patients in their own care and set SMART goals.
• Identify barriers to learning and care delivery and develop plans with providers.
• Independently manage caseload and ensure timely documentation for regulatory requirements.
• Coordinate patient information and referrals to internal/external resources for smooth transitions.
