Job summary

AHN Healthcare@Home in Connellsville, PA is seeking a Home Health RN to provide skilled nursing care to patients in their homes. This role includes a $10,000 first year bonus for achieving key milestones and opportunities for ongoing rewards as you grow with the company.

You will travel to patient homes within your territory, collaborating with an interdisciplinary team and documenting care in the EMR. A valid RN license and at least one year of experience are required.

Qualifications

• Active Registered Nurse, RN license in the state you work.
• One year of RN experience and the clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for.
