Allegheny Health Network is seeking a Home Health RN in the Apollo, PA area to deliver skilled nursing care in patients’ homes. You will collaborate with an interdisciplinary team, assess patients, manage medications, and educate families to ensure informed care plans.

Qualificações

• Active RN license in the state you work.
• At least 1 year of RN experience delivering home health care.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
