Job summary

Residential Home Health and Hospice in Clarks Summit, PA is seeking a Registered Nurse (RN) for home health visits to provide skilled nursing care in patients' homes. The role offers autonomy, meaningful patient relationships, and a first-year $10,000 success bonus.

You'll perform patient assessments, medication management, care planning, and education for patients and families, traveling within your territory and documenting care in the EMR.

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations
